Medicine Matters Home Patient Care Epic Tip of the Week: September 18, 2020

Epic Tip of the Week: September 18, 2020

Inpatient: Medications’ Last Dose Given appears in IP After Visit Summary [Effective October 3, 2020]

  • Inpatient clinicians view the last time a medication was given on the Preview AVS window
  • If a medication displays with a “____,” go to the Add Med Details section of the respective discharge navigators to edit
  • Clinicians view the last 7 days of recent doses in this section (will not show on the AVS)
  • If the system is unable to determine when the last dose was given, e.g. ibuprofen 800mg, click the pencil to enter the appropriate dose (e.g. Current date 9/9/20 at 1058)

Ambulatory: Automatically Re-route In Basket Messages to Covering Provider [Effective October 3, 2020]

When a provider leaves JHM, use the “Departing Provider Active Care” report in Reporting Workbench to assign a single covering provider to all categories of a patient's follow-ups (A), or assign separate covering providers to the individual categories of a patient's follow-ups (B).

Inpatient & Ambulatory: Updates to COVID-19 Orders [Effective September 17, 2020]

  • To assist Johns Hopkins with meeting Federal reporting requirements, the following additions were made:
    • Required questions:
      • Added “The patient is a frontline JHM healthcare worker, if “Is the patient a healthcare worker?” is answered “Yes”
      • Added the date of symptom onset, if “Is the patient having COVID symptoms?” is answered “Yes”
      • Whether or not the patient is currently hospitalized, the answer is defaulted “No” for ambulatory and ED patients and “Yes” for inpatients
      • Whether or not the patient is in an ICU, the answer is defaulted “No” for ambulatory patients, but left blank for ED and inpatients
      • Whether or not the patient resides in a group care setting
  • If the patient is asymptomatic, added options:
    • For “IRB Required Testing,” the selection should be either:
      • Required per JHM testing algorithm/clinical care guidelines
      • Sponsor/IRB is requiring COVID-19 testing to initiate research enrollment
    • For “JHM Approved Travel,” patient must meet criteria for approved travel, per the JHM Travel Policy

Inpatient: Updates to Tobacco Abstinence Panel [Effective September 15, 2020]

  • The adult Tobacco Abstinence order panel reflects the new policy for JHH and JHBMC to be smoke-free campuses
  • This panel is embedded in key admission order sets
  • If a patient is documented to be an active smoker in the Tobacco or E-Cigarette section of the Social History, the Nicotine Replacement section will be “open” in the order set, and red text noting the patient’s smoking status will display
  • If the panel is selected, educational text will display recommended treatment and questions for indicating the number of cigarettes smoked per day; each selection generates orders appropriate for that level of nicotine dependence
