Jae Patton is a nurse practitioner in the Palliative Medicine Section of the Division of General Internal Medicine. With over 25 years as an advanced practice clinician and leader, Jae has made significant contributions across various areas of cardiology and cardiac surgery in the Baltimore area. Since joining the Palliative Medicine program at Johns Hopkins Hospital in the spring of 2021, Jae has seamlessly transitioned into this new field, demonstrating a natural ability to bridge the intricate worlds of cardiology and palliative medicine at a time when such collaborations are only beginning to gain national attention. In her role within Palliative Medicine, Jae has led significant cultural change by integrating cardiology and palliative care. She has been instrumental in establishing a Palliative Medicine Advanced Cardiology Care Clinic within the Heart Failure Bridge Clinic and a Palliative Medicine Clinic within the LVAD/Heart Transplant specialty. Her advocacy has not only improved patient satisfaction and engagement but has also led to reduced hospitalizations and shorter lengths of stay for patients who typically require extensive care.
Amy Wenz is a nurse practitioner in the Division of Endocrinology, Diabetes & Metabolism. Throughout her tenure with the Inpatient Diabetes Management Service, Amy has shown a willingness to take on additional responsibilities such as training internal medicine residents, fellows, trainees and other nurse practitioners. She has mastered the complexities of inpatient diabetes care, which requires expertise in managing insulin pumps, automated insulin delivery systems and understanding the effects of various factors such as renal function and nutritional intake on glucose levels. Amy’s ability to recognize both common patterns and unusual presentations in diabetes care is critical to delivering effective and accurate treatment.
